Have you received any information from your child’s school or the Department of Health regarding the H1N1 or Seasonal Flu vaccines? How did you get the information?

    Yes. Notices from the school were sent home with my child.

    Yes. Information is posted on the school website and parent resource page. Notices were sent home and an electronic call was made to all parents.

    My son’s school was the site for the vaccine and he also received information from his fitness instructor.

    YES, I RECIEVED INFORMATION FROM THE DEPT OF HEALTH AT MY CHILD SCHOOL. MY DAUGHTER BROUGHT IT HOME.

    Yes I have received information about the swine flu vaccine from my children school via a Pamphlet.
